1. Legal and Physical Custody

Both parents shall have joint legal custody meaning the authority to make decisions regarding the child's health, education, and welfare.

2. Visitation Schedule

The non-custodial parent shall have visitation rights as follows:

• Days of the week or month, e.g., alternate weekends from Friday 6:00 PM to Sunday 6:00 PM
• Holidays shall alternate yearly, unless otherwise agreed in writing.

3. Transportation Responsibilities

The parent beginning the visitation period shall be responsible for transportation, unless otherwise agreed.

4. Child Support and Expenses

Each parent shall contribute to the child's support according to state child support guidelines or as otherwise mutually agreed in writing.

Additional expenses, such as medical, educational, or extracurricular costs, shall be shared equally.

5. Decision-Making Authority
